She?s a grand old girl, but she's off on a dirt road and badly needs some immediate fine-tuning, an oil change and possibly a new direction in some of the personnel charged with keeping her going."
If one was on, say a train, and it had come off it's tracks....I'm thinking that the wreck has already happened and the situation is what we might call 'dire'. Urgency would be the order of the moment.

If one was on, say a boat, and lets add the fact that it had no rudder. Steering might be an issue, strength of hand notwithstanding. Now, lets keep going and lets say the grand old girl with no rudder found itself on a dirt road. And the strong hand attempting to steer the rudderless vessel asks his 1st mate "what do we need?" If Gilligan were to respond with "some immediate fine tuning and and oil change" the new direction in some of the personnel keeping her going would be a pretty good idea.
