Pretty sure in the case of AIDS it'd be something like:
acute cause: pneumonia
underlying condition: AIDS
covid would be something like:
acute cause: pneumonia
underlying condition: COVID
covid + cancer would be something like:
acute cause: pneumonia
underlying condition: COVID
underlying condition: cancer
I could be totally wrong though, I don't know this for a fact
