Someone was killed in Arkansas last spring when a the door to a tornado safe room failed.
Here is a research study on what happened:

Looks like the safe room was well made, but they skimped on the door and the door hardware.
Recommendations from the report:
Site constructed tornado safe rooms should be constructed in strict accordance to FEMA P-320
Safe Room doors should be FEMA 320 ?Tested Door and Hardware Assemblies?

Door manufacturers should provide notices to resellers and wholesalers of the dangers of selling non-tested door products for tornado safe rooms.
