He has 301 career saves and about 40 blown saves. Happens. Even Mariano had only 90% success rate for saves.
Phillies are about to win this game with David Carpenter going out for another inning.

I think he's got 2-3 more 4 tops so it'll be pretty tough to get to 400. His biggest obstacle imo is a team trusting him enough to be the closer in a couple years. But he could get lucky and a team like the Astros or Padres will sign him for a discounted rate once his Phils fortune runs out and he can be the closer. Lots and lots of teams would love to have Papelbon right now I don't care how much the Phils fans bitch about him. He still has less than a 1.80 era. Joe Nathan for example has a 6.57 era. Sergio Romo just blew back to back saves giving up 8 runs. Paps will have a job as long as he wants if he's converting 90% of his opps and has a below 2 era.
